The following are the questions that are answered on the survey forms for each state:

1. State Surveyed?
2. Does the state require fire sprinkler contractors to be licensed?

a. If yes, what is the initial fee?
b. Is there an annual renewal requirement?
c. If there is, what is the amount?
d. Is there a CEU requirement?
e. If yes, how many?
3. Does the state have different levels or classes of licenses?

4.Does the state recognize NICET?
a. If yes, what levels?
5. Does the state conduct its own examination?
a. Is there an exam fee?
6. Does the state require a PE of FPE seal on all plans?
7. Does the state allow for Responsible Managing EMployees (RME's)

9. Does the state have insurance requirements?
a. If yes, what kind and amount?
10. Does the liscens law allow for reciprocity with other states?

11. Are there any penalties for noncompliance (working without a valid liscense)?
12. What state agency is responsible for the licensing of fire sprinkler contrators?
